AISL Blood Drive (Student Permission for those 16 and older)

We will be hosting a blood drive here at AISL next week Wednesday, November 22 from 9am - 3pm in the Pavilion. This is a fantastic opportunity for our students who are 16 or older to also participate and help to save lives. In order for students to donate who are 16 or older we need parents/guardians to complete THIS SHORT FORM to provide permission. To date we only have 15 responses from parents.
